In Ohio, you can choose someone you already know and trust to be your caregiver. This can include:
Yes, you may qualify if you have Medicaid, are approved for the PASSPORT Waiver, and are 60 or older. You must also live in the Cleveland area and be able to direct your own care or have someone you trust to help.
For MyCare Ohio, you must be enrolled in MyCare Ohio, which requires being 21 or older and having Medicare and full Medicaid, and then separately qualify for the MyCare Ohio waiver.
Sometimes, yes, but spouse cases are restricted. For both PASSPORT and MyCare Ohio, a spouse may provide HCBS waiver services only if no other willing and able provider is available and the state or its designee determines the member’s health and safety can be ensured per the state Rules.
Often, yes. An adult child can potentially be the paid SFC caregiver for a parent in PASSPORT or MyCare Ohio if the adult child lives with the parent, the parent qualifies for the waiver and for SFC, and an approved agency hires or contracts with the adult child as the caregiver.
Yes. Both programs are Medicaid waiver pathways. PASSPORT is Ohio’s Medicaid waiver for older adults, and MyCare Ohio combines Medicare and Medicaid benefits for eligible dual-eligible members.
To apply for the PASSPORT waiver, you’ll start with a pre-admission screening through your local Area Agency on Aging. If your loved one appears eligible, a case manager will visit to assess their needs and create a care plan. FreedomCare can walk you through each step and help with the paperwork so you can get approved and start receiving caregiver pay.
For MyCare Ohio, you’ll need to be eligible for the MyCare Ohio plan itself, which generally means being 21+, having Medicare and full Medicaid, and living in a county where the program is available. Ohio directs people to compare/select plans through the Ohio Medicaid Consumer Hotline and its portal; enrollment can be done online or by calling 800-324-8680. After you’re enrolled in a MyCare Ohio plan, you can be evaluated for the MyCare Ohio waiver, which requires nursing-facility-level care and the other waiver criteria.
Ohio SFC is a waiver service that pays an approved agency to support a live-in caregiver arrangement for an eligible Medicaid waiver participant who needs ongoing daily care at home.
